Application to the Chairperson of the Construction Contracts Adjudication Panel for Appointment of Adjudicator

This form may be used to apply to the Chairperson of the Construction Contracts Adjudication Panel. A party to a construction contract may apply to the Chairperson to have an Adjudicator appointed to a payment dispute pursuant to Section 6(4) of the Construction Contracts Act, 2013. The payment dispute must have arisen in relation to a construction contract entered into after the 25th July 2016.

If an applicant is applying to have an Adjudicator appointed by the Chairperson to a number of payment disputes arising under the same construction contract, separate applications must be submitted for each individual payment dispute.

An applicant may apply to the Chairperson using an Electronic version of the application form. If this option is selected the applicant will be requested by the Construction Contracts Adjudication Service to attach the accompanying documents using a secure web link.

An applicant also has the option of submitting the application with the accompanying documents, using the Word version of the application form, by post to the address at the bottom of that form. The application may also be submitted by fax to the number shown at the bottom of the Word version of the application form.

The applicant is required to send a copy of the application form and the accompanying documentation to the Respondent Party/Parties at the same time the application is submitted to the Chairperson, whether the Electronic version or the Word version of the application form is used.
